Organization Event


(Drop down list)

Use the Drop-Down List to choose the type of organization you are entering. Then use TAB key "or" MOUSE to move to next field.
Performing Group Type

Use the Drop-Down List to choose the type of group (Varsity, Non-Varsity, etc). Then use TAB key "or" MOUSE to move to next field.
Contest Name

Use the Drop-Down List to choose the contest you are entering. The exec sec would normally have all contests "setup" so all you would have to do is choose the appropriate contest. IF the contests are not setup, you will need to enter the DATE and DEADLINE for the event you are entering.
Contest Date

There is a Drop-Down Calendar for you to use to enter the DATE of the event if the contests are not listed.
Deadline Date

You do NOT need to use
$ sign. When you finish entering the info and click on PRINT, the FORM 1 will appear and the appropriate fees will be on the form "with" the $ sign.

The box is a "container box" and the sample names need to be removed. You can prepare your list in another program (word processor, etc), then copy and paste into the box OR you can leave this box "empty" and attach a separate list (class roster, etc) of respective students to the FORM 1.

For FORM 1, the students grade in school is not necessary. Most regions require that the names be "alphabetical" - last name, first

Click on "Get Form" button! The FORM 1 will appear and can be printed and mailed to the respective person. And that's all folks with regard to preparing a FORM 1.
